The following services are included with your annual fee:

• Two Regular Dental Check Ups:
  • Exam, cleaning, digital X-rays (if needed), oral cancer screening, oral hygiene instruction, periodontal evaluation, TMJ screening, and Fluoride treatments (if needed). Regular fee: $608.00
• Unlimited Emergency Visits:
  • Problem-focused exam, X-rays (if needed), diagnosis, and treatment options. Regular fee: $175.00
• Up to 15% off our Usual & Customary Fees:
  • A treatment plan will be provided in writing for all procedures, displaying the regular fee and the plan fee.*

*All treatment must be paid in full at the time of service to receive a discount. Cannot be combined with any other offer. If you choose to use extended payment plans, the usual/ customary fee is reduced by 5%.

• Family members cannot substitute for one another.
• Your effective date begins the day you sign up; your renewal occurs 364 days from that date. Members will be notified one month before membership renewal.
• You may choose to have your enrollment fee automatically renewed annually by charging the credit card on file.
• Cleanings must be completed before the end of the coverage period.
• This is a dental membership plan, not dental insurance, and it cannot be combined with insurance.
• Membership dues are non-refundable.
• Membership fees are only valid at SmilesByGlerum.com.
• Membership fees apply to services, not products.
• Payment for services is due at the time of service. If you use Care Credit or another extended payment option, the usual/customary fee is reduced by 5% due to merchant processing fees.
• A cancellation fee applies if you provide less than 24 hours’ notice for any appointment.
